:5May2008ToPrincipals/Persons-in-charge,RecentriseinHand-foot-mouthDisease(HFMD)andenterovirus71(EV71)activityFurthertomyletteralertingyouontheHFMDsituationson31March,IwouldliketourgeyoutoensureadequatepreventivemeasuresareinplacetopreventanypotentialspreadofHFMD,especiallythosecausedbyEV71,inyourschool.
Sofar,thisyear,wehaverecorded20institutionaloutbreaksofHFMD,with7outbreaksoccurringinthepastfourweeks.
Mostoccurredinkindergartenandchildcarecentres.
WehavealsodetectedmoreEV71cases,atotalof10patientssofar,ascomparedwiththesameperiodinthepastthreeyears.
IncreaseinEV71activity,especiallyseverecases,inneighbouringareas,includingSingapore,Taiwan,andsomeareasintheMainlandalsodeservesgreatattention.
HFMDisaviralinfectioncommonlyseeninchildren.
ThemainsymptomsofHFMDarefever,sorethroatandskinrashoverthehandsandfeet.
Althoughtheillnessisself-limitinginmostcases,somecasesmaybecausedbyEV71virusandmaybeassociatedwithcomplicationslikemyocarditis,encephalitisorpoliomyelitis-likeparalysis.
NospecifictreatmentisavailableforHFMD.
GoodhygienepracticesarethemostimportantmeasurestopreventHFMDandEV71infection.
TopreventHFMDoutbreakandEV71infectioninyourinstitution,Iwouldrecommendyoutakingthefollowingsteps:1.
Maintaingoodcommunicationwithparentsandhelpthemtounderstandtheneedtokeepchildrenathomeforrestwhentheydevelopfeverandrash.
AdvisethemtoseedoctorpromptlyiftheydevelopsymptomssuggestiveofHFMD.

ChildrenwithHFMDshouldbeexcludedfromschoolsuntilfeverhassubsidedandallthevesicleshavedriedandcrusted.

Supervisechildrenonpersonalhygienepracticesespeciallyonhowtowashhandsproperlywithliquidsoapbeforemealsandaftergoingtotoilet,covernoseandmouthwhilesneezingorcoughing,andproperdisposalofnasalandmouthdischarge.

Keeptheenvironmentcleanandhygienic,andfollowtheGuidelinesonPreventionofCommunicableDiseasesinChildCareCentres,Kindergartens,andSchoolswhichisavailableathttp://www.

Lastbutnotleast,pleasealsoincorporateinyourhealtheducationtochildrenthat,whenthechildrenarehavingholidaysormeetingwithrelativesoutsideHongKong,theyshouldalsoremembertokeeptheirhandscleanandavoidtouchingvesiclesonhandsorotherbodilyareasofothersickchildren.

twoormorechildreninthesameclass/activitygroupdevelopHFMDwithinaperiodof7days),pleaseinformtheCentralNotificationOfficeofCHPasearlyaspossibleatFax:24772770orTel:24772772.
CHPwillgiveadviceonthemanagementofsuchoutbreaksandtakeappropriatecontrolmeasures.
Duringoutbreaks,pleasemaintainclosecommunicationwithparentsontheconditionofthechildrenandreporttoCHPiftheaffectedchildrenhavebeenadmittedtohospitals.
Wemayconsidertoadviseschoolsaffectedtosuspendclassesforaperiodof14dayswhensituationwarrants.
ExamplesofsuchscenariosincludeoccurrenceoffurthercasesinaninstitutionattendedbyaconfirmedEV71caseaftersteppingupcontrolmeasures,ORoccurrenceofanEV71casewhoissufferingfromseverecomplicationandtheinstitutionthechildisattendinghasanassociatedHFMDoutbreak.
AsthevirusmaybeexcretedinstoolsforsomeweeksandEV71isassociatedwithahigherriskofcomplications,asanextraprecaution,achildwhohasEV71infectionisadvisednottogotoschoolforanothertwoweeksafterallsymptomshavesubsided.
